SoftWave Therapy: The Needle-Free Joint Pain Breakthrough Everyone Is Asking About
For years, many of my patients with aching knees, stiff shoulders and sore heels heard the same three options: take another pill, get another injection, or wait until it gets bad enough for surgery. I understand that frustration. Pain wears on your body, and it wears on your spirit too. It quietly steals the morning walk, the round of golf, the afternoon in the garden and the simple joy of getting down on the floor to play with your grandchildren.
That is why I am so excited about a therapy we now offer at the Colbert Institute in Lake Mary, Florida and Southlake, Texas. It is called SoftWave therapy, and it is one of the most fascinating advances in joint and soft tissue rehab I have seen in years. No needles. No incisions. No anesthesia. Little to no downtime for most people. Just a gentle approach designed to work with the body God created rather than against it.
In this article I will walk you through what SoftWave is, how it differs from older devices, what the research actually shows, and who may be a good candidate. I will be honest about what we know and what we are still learning, because you deserve both hope and the truth.

🌊 What Is SoftWave Therapy?
SoftWave is a non-invasive therapy that sends acoustic (sound wave) energy through the skin and into painful or injured tissue. Its purpose is not to numb the area or cover up the pain. Its purpose is to wake up the body’s own repair process in tissue that has become stuck in a cycle of irritation and slow healing.
SoftWave comes from the same scientific family as extracorporeal shockwave therapy, which medicine has used for more than 30 years, first to break up kidney stones without surgery. But SoftWave is a very different experience from the older devices many people have heard about. Its patented applicator spreads a broad, unfocused wave across the treatment area instead of concentrating all the energy on one tiny point. Many older focused devices were designed to create small amounts of controlled tissue irritation to provoke healing, which can be quite uncomfortable. SoftWave is designed to deliver the healing signal without damaging tissue, which is a big reason so many patients find it easy to tolerate.
Is SoftWave FDA Cleared?
Yes. In May 2021 the FDA cleared the SoftWave OrthoGold 100 device (510(k) number K210451) for a temporary increase in local blood circulation, relief of minor aches and pains, and activation of connective tissue, according to the company’s announcement. The same technology has also received separate FDA clearances in wound care, for diabetic foot ulcers and superficial second-degree burns. “Cleared” is the FDA’s term for medical devices like this one, and it is a different process from how drugs are approved.
💡
Did You Know?
The same kind of sound wave energy that once replaced surgery for many kidney stone patients is now being used in a gentler form to support joints, tendons and connective tissue. SoftWave simply takes that proven physics in a regenerative direction.
⚙️ How SoftWave Supports Your Body’s Natural Repair
I like to describe SoftWave as a wake-up call for tissue that has fallen asleep on the job. Areas of chronic pain often have poor blood flow, lingering inflammation and stalled healing. Research on low-energy acoustic waves describes several ways the body responds:
- Cell signaling (mechanotransduction): your cells sense the mechanical energy of the wave and translate it into chemical messages that switch on repair activity.
- New blood flow: studies show acoustic waves increase VEGF and nitric oxide signaling, two of the body’s key messengers for building new tiny blood vessels (a process called angiogenesis). Better circulation means more oxygen and nutrients reaching tired tissue.
- Stem cell activity: research reviews describe acoustic waves helping recruit and activate the body’s own repair cells, including mesenchymal stem cells.
- Calming inflammation: acoustic waves appear to help modulate the immune response, nudging chronic irritation toward resolution.
- Tissue remodeling: the support structure around your cells, including collagen, is encouraged to reorganize and strengthen.
I want to be clear that much of this mechanism research comes from laboratory and animal studies, as summarized in reviews like this one on the molecular pathways of shock wave therapy. Still, it paints a beautiful picture: rather than overriding the body, this therapy appears to send it a signal to do what it was already designed to do.
“Pain is a signal, not a life sentence. When we give the body the right signals, I am often amazed at how faithfully it responds.”

🔬 What the Research Shows for Joints and Soft Tissue
Knee Arthritis
Knee osteoarthritis is one of the best studied areas. A large systematic review and meta-regression pooled 50 randomized trials with 4,844 patients. It found that acoustic wave therapy was linked to about three times higher odds of treatment success, along with meaningful improvements in pain and physical function. The researchers also found that the energy level and length of treatment mattered, which is why a well-designed treatment plan is so important.
A separate 2020 meta-analysis in the International Journal of Surgery reported lower pain scores at 4, 8 and 12 weeks and improved knee function, without an increase in side effects. And an umbrella review that combined eight meta-analyses concluded the therapy is effective for improving pain and function in knee arthritis.
Tendons, Heels and Elbows
A 2024 meta-analysis of randomized trials found pain reductions for plantar fasciitis (that stabbing heel pain with your first steps in the morning), tennis elbow and chronic Achilles tendon pain. An earlier review in the British Journal of Sports Medicine found good evidence for plantar fasciitis and calcium deposits in the shoulder tendons, and noted that the therapy was safe.
An Honest Look at the Limits
Most published trials used focused or radial shockwave devices, not SoftWave specifically, and the devices, energy levels and number of sessions varied widely from study to study. Results for some conditions, such as tennis elbow, have been mixed. Not every person responds, and SoftWave works best as part of a complete plan that includes movement, nutrition and rest.
That is exactly why we evaluate each patient individually before recommending a plan.
👥 Who Might Benefit From SoftWave? Real-Life Examples
Here are the kinds of people I picture when I think about SoftWave. These are illustrative examples, not individual patient stories, but you may recognize yourself or someone you love in one of them.
🦵
The Grandparent With Stiff Knees
Stairs feel steeper every year and sitting on the floor with the grandkids is out of the question. Knee arthritis is where acoustic wave research is strongest.
🏌️
The Weekend Golfer’s Elbow
A nagging ache on the outside or inside of the elbow that flares with every swing, handshake or coffee cup. Overused tendons are a classic reason people explore SoftWave.
👣
The Morning Heel Pain Walker
Those first steps out of bed feel like stepping on a tack. Plantar fasciitis is one of the most researched uses for acoustic wave therapy.
💪
The Stubborn, Sore Shoulder
Reaching for the top shelf or fastening a seat belt hurts, and sleeping on that side is impossible. Shoulder tendon problems are a common reason patients ask about SoftWave.
🏃
The Active Adult With Hip or Back Tightness
Pickleball, hiking or yard work leaves the hips and lower back tight and achy for days. SoftWave may support muscle and soft tissue recovery.
🔄
The Sports Strain That Won’t Quit
An old pulled muscle or tweaked tendon that never fully healed. Lingering soft tissue injuries and recovery support are a core reason SoftWave exists.
At the Colbert Institute, patients commonly come to us about joint discomfort, muscle or tendon pain, soft tissue injuries, sports-related strain, chronic inflammation, limited mobility, plantar fasciitis and foot pain, and shoulder, knee, hip, back or elbow discomfort.

🩺 What to Expect at Your SoftWave Visit
- A conversation first. Our team reviews your symptoms, your health history, your goals and the area to be treated.
- A little gel. A gel is applied to the skin so the acoustic waves travel smoothly into the tissue.
- Your feedback guides the treatment. As the applicator glides over the area, some spots may feel more tender or responsive. You tell us, and that helps us focus on the tissue that needs it most.
- A short session. Visits are quick, and most people go right back to their normal day.
- A personal plan. Depending on your condition and how your body responds, we may recommend a series of visits. Some people notice a change quickly, while others improve gradually as the body continues its repair work.
Side effects are usually minimal. Some people notice mild redness or soreness in the treated area that typically fades within a day or two.
Who Should Talk With Us Before Treatment
SoftWave is not right for everyone. Before treatment, please let our team know if you are pregnant, take blood-thinning medication or have a bleeding disorder, have an implanted device such as a pacemaker, or have a history of cancer in the area to be treated. And if you have a brand-new injury, severe swelling, a hot and red joint, fever, numbness or weakness, please see a doctor promptly so the cause can be properly evaluated.
🌿 Supporting Your Joints Between Visits
SoftWave can be a powerful part of the picture, but I always want my patients to build a foundation that supports healing every single day.

✦ Dr. Colbert’s Faith Perspective
Designed to Heal
Every time I see a cut close up or a sprained ankle recover, I am reminded that healing is written into our design. The Creator placed repair crews in every tissue of your body. The best therapies do not replace that design. They honor it and give it room to work.
If chronic pain has made you feel older than your years, do not lose heart. Pray over your body, take wise steps and trust God with the process.
“For I will restore health to you and heal you of your wounds,” says the LORD. (Jeremiah 30:17, NKJV)
