Keto for Weight Loss: Same Pounds Lost, 67% Less Liver Fat

Keto for Weight LossNew ResearchMediterranean KetoMuscle Over 50MCT OilHealthy FatsFood First

Turn on the evening news or scroll through your phone, and it can seem as if the whole world is talking about weight-loss shots. Friends compare notes at church. Neighbors ask each other which one they are on. In all that noise, a simple question gets lost: does what you put on your plate still matter?

In my practice, patients ask me almost every week whether keto for weight loss still makes sense in this new era. Now a carefully controlled clinical trial gives us a fresh, encouraging answer. The food you eat can change how well your body responds to losing weight, even when the number on the scale is exactly the same.

Researchers at Washington University School of Medicine in St. Louis put three very different diets head to head. When everyone lost about the same amount of weight, the ketogenic group saw the biggest drop in liver fat and the strongest shift in insulin, without cholesterol getting worse. You can read the summary on ScienceDaily. Let me walk you through what they found, what it does not prove, and how to turn it into a wise, food-first plan.

67%average drop in liver fat on keto
45%drop on Mediterranean and plant-forward diets
10%body weight lost in every group

πŸ”¬ What the New Keto for Weight Loss Trial Found

This was a randomized clinical trial in people, published in the journal Cell Metabolism and funded by the NIH and the Foundation for Barnes-Jewish Hospital. “Randomized” means participants were assigned to a diet by chance, which helps make the comparison fair. Every meal was supplied, and participants met weekly with a dietitian.

The volunteers were adults with obesity, prediabetes and fatty liver disease. Forty-two people completed the study (some coverage reports that 55 were randomized at the start). Each person followed one of three diets:

  • Very-low-carb keto: 4% of calories from carbohydrate and 73% from fat.
  • Mediterranean: 50% carbohydrate and 35% fat.
  • Low-fat, plant-forward: 70% carbohydrate and 15% fat.

Here is the clever part. Calories were adjusted so that every group lost about 10% of body weight, which took roughly 4 to 5 months. Because the weight loss was matched, the researchers could ask a sharper question: with the same pounds gone, does the kind of food make a difference?

The good news first: all three diets worked. Total body fat fell significantly in every group, and insulin sensitivity (how well your cells respond to the hormone insulin) improved in all three. But keto stood out in several ways:

  • Liver fat fell by an average of 67% on keto, compared with 45% on both the Mediterranean and plant-forward diets.
  • The keto group had the largest rise in glucagon (a hormone that tells the body to release stored fuel) and the biggest drop in blood insulin over 24 hours.
  • About 50% of the keto group no longer met the criteria for prediabetes, compared with 29% on the Mediterranean diet and 7% on the plant-forward diet.
  • Cholesterol did not worsen on keto, even with the highest fat intake. First author Petersen called this the most surprising finding.

According to secondary coverage, total cholesterol, LDL and apoB (a marker of the particles that carry cholesterol) fell after weight loss, with no difference between the groups. Petersen also noted that while GLP-1 medications have changed obesity treatment, the makeup of the diet itself can bring added benefits.

The Honest Limitations

I always want you to see the whole picture. This trial was small and lasted under 5 months. Every meal was prepared for participants, which is far easier than shopping and cooking on your own. Everyone in it had obesity, prediabetes and fatty liver, so the results may not apply to healthy adults.

Most importantly, because calories were set to equalize weight loss, this study does not show that keto makes you lose more weight. It also did not report hunger or muscle outcomes. One study is never the final word, but it is a meaningful piece of the puzzle.

🧬 Why Keto Changes How Your Body Burns Fat

Your body is fearfully and wonderfully made, with a beautiful system for switching fuels. When you eat a lot of carbohydrate, insulin rises. Insulin is a storage hormone: it tells your body to tuck energy away and keeps fat locked in place.

When carbohydrates drop very low, insulin falls and glucagon rises. That combination signals your body to open the pantry and burn stored fat. Your liver begins turning fat into ketones, small fuel molecules your brain and muscles can use. The main one is called beta-hydroxybutyrate, or BHB.

That is exactly the hormone pattern the Washington University team measured: the biggest insulin drop and largest glucagon rise in the keto group. It makes sense that the liver, the very organ that handles this fuel switch, would show the most dramatic change.

πŸ’‘

Recent Research

A separate meta-analysis of randomized trials found keto and low-carb diets significantly reduced fat mass, and described that fat loss as coming mainly from higher fat burning rather than loss of lean muscle. It also cites work suggesting keto may reduce visceral fat (the deep fat around the organs) more than the fat just under the skin.

What about hunger? An earlier National Lipid Association scientific statement concluded that low-carb and very-low-carb diets are not superior to other approaches for weight loss itself. However, they may have advantages for appetite control and lowering triglycerides, and people often report less hunger in ketosis, even though the reasons are not fully understood. That is one of the most natural ways to curb appetite I know: let your own fuel system steady you.

πŸ’ͺ Keto Diet and Muscle Loss: A Food and Strength Job

Here is a part of the weight loss conversation I care about deeply for my patients over 50. When you lose weight, you rarely lose only fat. You also tend to lose some muscle, and muscle is your metabolic engine, your balance and your independence as you age.

This matters in the GLP-1 era. A meta-analysis in the International Journal of Obesity found a significant drop in lean mass at obesity-management doses, about 1.74 kg, or roughly 3% of lean mass. Its authors note that roughly 25% of weight lost on these drugs corresponds to lean mass. A companion review advises pairing therapy with progressive resistance training and adequate high-quality protein. If you are on one of these medications, please do not stop or change it on your own. Talk with your doctor, and you may find my thoughts on medical supervision with GLP-1 medications helpful.

Whether you use medication or not, protecting muscle comes down to the same two things: your fork and your weights. An explainer from Rachel Woods of the University of Lincoln, shared on ScienceDaily, notes that losing muscle lowers the calories you burn at rest, which can contribute to regain. Resistance training can help preserve or rebuild muscle, and in a study of over 1,100 people, higher activity after weight loss was strongly linked to keeping the weight off.

Food quality matters for muscle, too. In MRI data from 615 adults averaging 60 years old, eating more ultra-processed food was linked to more fat tucked inside the thigh muscles, even after accounting for calories, fat intake and activity (ScienceDaily/RSNA). That study was observational, so it shows a link, not cause and effect. Still, it reminds us that a bag of “keto” snack bars is not the same as a plate of real food. For more on staying strong, see my article on creatine and muscle in older adults.

πŸ«’ The Cholesterol Question: Build Keto on Better Fats

The fact that cholesterol did not rise in this trial is reassuring. But I would not be honest with you if I stopped there. Different people respond differently to dietary fat, and genetics plays a role.

New findings from the DIETFITS trial, presented at NUTRITION 2026 and covered on ScienceDaily, suggest a low-carb diet may raise LDL cholesterol much more sharply in people genetically predisposed to high cholesterol, particularly when it contains lots of saturated fat. Stanford’s Alexa Barad explained that some people are simply more sensitive to saturated fat’s LDL-raising effect. The body composition meta-analysis I mentioned also noted a related paper in which keto produced greater increases in LDL and total cholesterol.

That is why I have steered my patients for years toward a Mediterranean-style keto: olive oil, avocados, nuts, wild fish and plenty of non-starchy vegetables, rather than piles of bacon and butter. I explain this approach fully in Dr. Colbert’s Beyond Keto, and you can find great everyday choices in 7 healthy fats to help you burn belly flab. Whichever way you eat, ask your doctor to check your lipids before you start and again a few months in.

πŸ₯₯ The Top 5 Ingredients for Food-First Keto

These are the building blocks I lean on most. Notice that the foundation is food. Supplements come last, as modest helpers, never as a replacement for a well-built plate.

πŸ«’

Unsaturated Fats

Because saturated-fat-heavy low-carb eating may sharply raise LDL in sensitive people, make these your main fats. Food sources: extra virgin olive oil, avocados, walnuts, almonds and olives.

🐟

High-Quality Protein

Researchers recommend adequate high-quality protein alongside weight loss to help protect lean mass. Food sources: wild salmon, sardines, pasture-raised eggs, poultry and grass-fed meats.

πŸ₯¬

Whole, Unprocessed Foods

More ultra-processed food was linked to more fat inside the thigh muscles. Food sources: leafy greens, broccoli, cauliflower, zucchini and fresh herbs.

πŸ₯₯

Medium-Chain Triglycerides (MCTs)

These shorter fats (C8 and C10) are converted to ketones in the liver. Meta-analyses show modest benefits for weight, waist and eating a bit less at the next meal. Food source: coconut oil.

⚑

Beta-Hydroxybutyrate (BHB)

BHB is the main ketone your body makes on keto. One review attributes part of keto’s fat loss to higher BHB, a mechanistic idea rather than proof for supplements. Source: your own body, during carb restriction or fasting.

MCT Oil for Keto: Modest but Real

A meta-analysis of randomized trials found that, compared with ordinary long-chain fats, C8 and C10 MCTs reduced body weight by about 0.51 kg (roughly a pound) and waist size by 1.46 cm (a little over half an inch) over about 10 weeks. That is small, and I want you to hear that honestly.

A second meta-analysis on satiety found MCTs led to a moderate drop in how much people ate at their next meal, although self-reported appetite and appetite hormones changed little. In plain terms, MCT may help you eat a little less later. According to a 2024 meta-analysis, MCTs are converted to ketones in the liver, which is why they fit so naturally into a keto lifestyle.

One important note: the products below were not tested in the Washington University trial, which studied whole-diet keto only. They are not intended for any medical condition. I recommend them simply as tools for energy, focus and satiety for healthy adults following a keto lifestyle.

Exogenous Ketones: A Convenience, Not a Substitute

Exogenous ketones are ketones that come from outside the body, usually as BHB in a drink. I want to be clear: I did not find a human trial showing BHB supplements curb appetite or protect muscle. Their role is to support ketosis and clean energy, for example during the first weeks of cutting carbs or between meals when you practice time-restricted eating. The real work still happens on your plate.

Dr. Colbert Recommends

Divine Health Instant Ketones Powder

Instant Ketones provides goBHB, a form of beta-hydroxybutyrate, the main ketone your body makes on a ketogenic diet. Many of my patients like it as a convenient companion while they transition into a keto lifestyle. It is designed to complement whole-food keto, not replace it.*

✦ Dr. Colbert’s Faith Perspective

Stewardship Before Shortcuts

I am grateful for every tool medicine offers, and I never want anyone to feel shame for using one under a doctor’s care. But God has also built remarkable wisdom into our bodies and into the food He created. In Daniel 1, Daniel chose simple vegetables and water over the king’s rich table, and at the end of ten days he and his friends appeared healthier than the others. That is a picture of food-first faithfulness.

Self-control is listed among the fruit of the Spirit in Galatians 5:22-23, and that includes the choices we make at the table. We do not change our habits in our own strength. We ask the Lord for grace one meal at a time, and we honor Him with the body He has entrusted to us. As John prayed in 3 John 1:2, “Beloved, I pray that you may prosper in all things and be in health, just as your soul prospers.”

“Or do you not know that your body is the temple of the Holy Spirit who is in you, whom you have from God, and you are not your own? For you were bought at a price; therefore glorify God in your body and in your spirit, which are God’s.” (1 Corinthians 6:19-20, NKJV)

“The scale tells you how much you lost. Your plate decides what kind of body you keep. Feed your muscles, choose God’s whole foods, and let your fat burning fire do what He designed it to do.”

βœ… Your Action Plan to Lose Fat and Keep Weight Off After 50

  1. Start with your doctor. Keto is not for everyone. If you take diabetes medication (such as insulin or SGLT2 inhibitors), blood pressure medication or a GLP-1 drug, or if you have kidney, gallbladder, pancreas or liver conditions, or are pregnant or nursing, get your physician’s guidance first. Ask for a lipid panel so you have a baseline.
  2. Build every plate on vegetables. Fill half your plate with leafy greens and non-starchy vegetables. They bring fiber, color and volume.
  3. Choose Mediterranean fats. Make olive oil, avocado, nuts, olives and wild fish your main fats, and keep saturated fat moderate, especially if high cholesterol runs in your family.
  4. Protect your muscle with protein. Include a palm-sized portion of quality protein such as salmon, eggs or poultry at each meal.
  5. Lift something two or three times a week. Resistance bands, light dumbbells or bodyweight squats all count. Strength work helps preserve muscle and keep the weight off.
  6. Cut ultra-processed foods first. Even “keto” packaged snacks belong in this category. Real food supports muscle quality, not just calorie counts.
  7. Use helpers wisely. A scoop of MCT Oil Powder in your coffee or Instant Ketones during your first weeks can support energy and focus. If you want a simple way to begin, the Keto Zone Starter Pack was put together for people starting a keto lifestyle; check the product page to see what is inside.
  8. Join the free challenge. Sign up for the free 21-Day Keto Zone Challenge for daily encouragement and structure.
  9. Recheck and pray. Have your labs repeated after a few months, and bring your goals to God in prayer. Progress, not perfection, is the goal.

❓ Frequently Asked Questions

Is keto better than the Mediterranean diet for weight loss?

Not for the pounds themselves. In this trial, calories were set so every group lost about 10% of body weight, and earlier expert reviews found low-carb diets are not superior for weight loss alone. Where keto stood out was in what happened inside the body, including a larger drop in liver fat and a bigger insulin shift.

Does keto cause muscle loss?

Any weight loss can cost some muscle. This trial did not measure muscle, but a separate meta-analysis described keto fat loss as coming mainly from fat burning rather than lean muscle. The best protection is adequate high-quality protein plus regular strength training.
